How to get an appointment
- To request an appointment, click the "Get Services from the CED" button below or call the CED at 304-293-4692 and request to speak with the clinic coordinator who can walk you through the Wayfinders referral process.
- Once we have received your referral for review and it has been determined that your service needs align within the scope of the clinic, a clinic coordinator will be in contact to ask any additional preliminary questions and to schedule a virtual meeting.
- Once a meeting date and time has been confirmed, we will send you an email with meeting details and video call information.
Day of clinic visit
On the day of the virtual clinic, you will enter the virtual meeting at the designated meeting time. The Wayfinders multidisciplinary team members will be on the video call along with the outside agency representatives relevant to your service needs. There may also be students present, but hidden from view, who are learning from the experience. The meeting may last as long as one hour and will be an opportunity to hear suggestions and insights from the team relevant to your service needs. You will have the opportunity to ask questions to any and all professionals present, make referrals, complete brief applications and explore programs as appropriate. You are encouraged to take notes; however, a summary of the visit will also be shared after the meeting.
The meeting will be recorded.
What to expect after visit
Within 5 days, you will receive the written summary from your meeting. This will include a summation of the discussion, any recommendations that were shared during the meeting, contact information for the outside providers for follow-up, and any other relevant links, applications, brochures, etc.
A clinic coordinator will be in touch within 2 weeks - 1 month following the clinic meeting to follow-up on service needs and to complete a satisfaction survey. If a follow-up meeting needs scheduled, this can be arranged at this time as well.
