We work with existing supports in an effort to assist in accessing services, meeting service needs and exploring options from a wholistic and multidisciplinary lens. If you are serving an individual with a disability or their family or caregiver, and feel that they could benefit from Wayfinders Clinic, you can refer them directly clicking the "Get Services from the CED" button, below, by calling the CED at 304-293-4692 and speaking with the clinic coordinator.
Once a referral has been submitted and reviewed, the clinic coordinator will be in contact with the family for any other preliminary questions and to schedule a virtual meeting.
The family will also be responsible for inviting you as the referral source or case manager, etc. to the meeting and for sharing the end of clinic summary unless they give our team explicit permission to do so on their behalf.
