How to get an appointment
-
To begin the appointment process, click the "Get Services from the CED" button below or call the CED at 304-293-4692.
- After talking with the Clinic Coordinator or Dr. Gilbert you will be emailed an Intake Form. This form provides us with basic information that will help us plan the consultation.
- Once we have received the intake form, we will call you to schedule your consultation or training support session. Our clinic operates each Tuesday, and the sooner we receive your forms, the sooner we can schedule the appointment.
- After the appointment date and time has been confirmed, we will send you our appointment packet. This packet includes confirmation of your scheduled appointment along with details about the clinic and directions to our location.
To be brought on day of appointment:
Alternate formats of the forms above are available upon request.
Day of clinic visit
On the day of the appointment, please bring the following items:
- Favorite snack items, toys or motivating activities that are easily transportable
- Current AAC system being used or previously tried
- Appropriate forms (see above) to be brought on day of appointment
When you arrive, follow the WVU CED Visitor sign to park in the top parking lot. The WVU CED is the middle building of the three tan buildings.
The clinical coordinator will inform the team that you have arrived at the center. A team member will greet you and guide you downstairs to the clinic where the appointment will take place.
Appointments typically take 30 minutes -1 hour to complete. Each appointment is unique based on communication abilities and needs. Please be prepared to answer follow-up questions and interact with different devices throughout the appointment. The team will complete their questions and observation, and the visit will end with a wrap up and recommendations.
Our team will follow up with you to share your summary report within one week of your appointment. If you have any further questions about the clinic, please email the clinic director Tori Gilbert at tori.gilbert@hsc.wvu.edu or call 304-293-4692.
What to expect after visit
After your visit, you can expect to receive an email from one of our professionals with an attached summary report reviewing the recommendations and resources discussed at your appointment. Once that email is distributed, a follow-up phone call will be placed to check in and give any additional information needed.
If you have any further questions about the clinic, please email tori.gilbert@hsc.wvu.edu.
